If you use 1 of the following 3 selections (Provider ID, License Number, or NPI), you will go directly to the provider information
you are searching for and other options you choose will be ignored for the search.
- Search by Provider ID:
- Enter the Provider ID and click on the Search button.
- Search by License Number:
- Enter the CA License Number and click on the Search button.
- Search by NPI:
- Enter the Provider NPI and click on the Search button.
To reduce the number of providers in your results list, choose more than one search option.
- Search by Name:
- Enter all or part of the Provider Last Name and/or all or part of the First Name and click on the Search button.
- Search by Location:
- Enter either the City name OR enter the Zip code, and distance and click on the Search button.
- Search by Primary Care Physician or Specialist:
- Select the type of Primary Care Physician and/or Specialist from the list and
click on the Search button. If you choose both types of providers, the resulting
list will be smaller than choosing just 1 type of provider.
- Search by Language Preference:
- Select the Language that the provider can speak and click on the Search button.
- Search by Sex (Gender) Preference:
- Select the Sex of the provider (Man / Woman / All) and click on the Search button.
- Search by Accepting New Patients:
- Select whether the provider is accepting new patients (Yes / No / All) and click on the Search button.
If you are a new patient to the provider and the provider is not accepting new patients, you may not be
able to make an appointment with that provider
- Search by IPA / Medical Group:
- Select the IPA / Medical Group from the list to see all of the providers who work in a specific Medical Group and click on the Search button.
- Search by Hospital Affiliation:
- Select the Hospital from the list to see all of the providers who have privileges for a specific Hospital and click on the Search button.
